Managing health care can be challenging at the best of times. For older adults with cognitive impairment or for people battling symptoms of exhaustion, navigating one’s own health care can be daunting. The result? They put off getting the care they need or do not seek care at all.
It is our mission to provide safe and healthy living options for all valley seniors regardless of income. Our patient navigators will meet with your family to determine what services are needed and what resources are best able to meet your care needs.
